As your hearing date approaches, the SEND tribunal will ask you to complete one or both of the following forms:
Attendance form (SEND11)
You use this to tell the tribunal who will be attending the hearing, including:
- you (parent or carer)
- your representative or supporter (if you have one)
- any witnesses (up to three)
You will receive directions telling you to complete SEND11, and submit the form by the deadline given. The tribunal uses the information to plan your hearing.
Case review form (SEND45)
The tribunal will send you this form if they need further information to help them understand:
- the current progress of the appeal
- which issues remain in dispute
- whether the case is ready to proceed to a hearing
- any special requirements needed at a hearing
Returning this form by the deadline is essential.
The tribunal may arrange a telephone case management hearing (TCMH) if more discussion is needed. Other changes could be decided by the tribunal at this points (for example a paper hearing or JADR)

- missing a case review form deadline may mean your witnesses are not allowed to attend
